PATH SITE VISITS

Monitoring and Promoting Effective Implementation of PATH-Funded Services

AHP conducts site visits for Projects for Assistance in Transition from Homelessness (PATH)-funded States or Territories each year to encourage the adoption of exemplary practices and identify TA needs of local PATH-funded providers. For each monitoring site visit AHP conducts, through a subcontract with AFYA, the site visit team writes a report of findings and recommendations and then follow-ups within six months of the site visit to monitor implementation of recommendations.  The AFYA/AHP team also has developed a trained cadre of site visit team monitors.  AHP provided formal training for monitors in January, 2005, and has continued to provide oversight and support as new monitors join teams. AHP worked with the Homeless Programs Branch of the Center for Mental Health Services (CMHS) to develop Voluntary Performance Goals for state administration of the PATH program and to develop a detailed protocol for the monitoring visits.
