TA to States and Local Providers Regarding the Projects for Assistance in Transition from Homelessness

The Projects for Assistance in Transition from Homelessness (PATH) formula grant program provides flexible, community-based services for people with serious mental illnesses and co-occurring substance abuse disorders who are homeless or at imminent risk of becoming homeless. AHP provided on-site TA and other activities to help States/Territories and local providers effectively operate and monitor the PATH program.  PATH TA was delivered to all 50 states and four territories on-site, by national teleconference calls, and by telephone. In addition, the PATH web site provided immediate availability to audio and text files of information provided by national experts, as well as a Bulletin Board for instant communication among PATH providers.  Under several PATH contracts, AHP developed a well-defined method to assess a State’s or provider’s needs, provide TA materials and finalize an assistance plan, and deliver the TA and any follow-up guidance that might be necessary.
