SERVICES If you have made an appointment to visit us, you can streamline your visit by filling out the pre-registration form online and submitting it directly to us. You may also print the form and fill it out to bring with you to your first appointment. Click on the link below to view a PDF file of our Pre-registration form. You will need the Acrobat reader (a free download) to do this. After you fill in the form, choose save from the Acrobat menu and save the file to your computer. You can then e-mail the file, as an attachment, to the addresses at the bottom of the form. If you prefer not to e-mail, you can print the form and fax it or bring it with you to your appointment.
